Social Worker Month
Picture this: You are a patient recently admitted into one of our many beautiful Sunterra Springs facilities with a new injury or diagnosis. You receive top notch nursing and therapy interventions and are now ready to transition to the next level of care. Whether this discharge scenario be to return home, senior housing, or an alternative environment so many options can make the process seem overwhelming. At Sunterra we rely heavily on the discharge expertise of our social workers.
As March is National Social Work Month, we wanted to take a moment to highlight the role of social work within our facilities. In our rehabilitation setting, our social workers work alongside the nursing and therapy staff serving as a patient advocate assisting with advanced care planning (durable power of attorney), resource education, and arrange for durable medical equipment as well as arrange for home health care/hospice or long-term care referrals. Their goal is to ensure our patients are discharged to the most safe and appropriate environment with any needed services and/or equipment in place.
